You will be a highly motivated and dedicatedNewly Qualified GP to join our multidisciplinary team under the NHS AdditionalRoles Reimbursement Scheme (ARRS).
Main duties of the job
Key Responsibilities:
1. Provide high-quality, patient-centred care across the PCN.
2. Work collaboratively with a multidisciplinary team, including nurses, pharmacists, mental health practitioners, and social prescribers, to ensure integrated patient care.
3. Conduct consultations, assessments, and follow-ups with patients, utilising ARRS-funded services to support patient needs.
4. Contribute to the management of chronic disease, health promotion, and disease prevention in line with best practices and PCN goals.
5. Participate in clinical audits, training, and quality improvement initiatives to enhance patient care within the PCN.
6. Support the development and implementation of new care pathways that align with ARRS objectives.

Clinical Responsibilities:
7. To provide a comprehensive range ofclinical duties at the agreed Surgeries within our Primary Care Network, whichmay also include working within our Enhanced Access Service.
8. To undertake surgery consultations, homevisits where necessary, and telephone queries from patients and health careprofessionals.
9. Making professional, autonomous decisionsin relation to presenting problems, whether self-referred or referred fromother health care workers within the organisation
10. To undertake on-call duties as part of theSurgery on-call rota, where agreed. This may include requesting that patientspresent at the surgery, telephone consultations, home visits and triage.
11. Work with patients in order to supportcompliance with and adherence to prescribed treatments. Provide information andadvice on prescribed or over-the-counter medication on medication regimens,side-effects, and interactions
12. Assess, identify, and refer patientspresenting with mental health needs in accordance with national and localguidelines.
13. Assessing the health care needs ofpatients with undifferentiated and undiagnosed problems
14. Screening patients for disease riskfactors and early signs of illness
15. In consultation with patients and in linewith current practice disease management protocols, developing care plans forhealth
16. Recording clear and contemporaneousconsultation notes to agreed standards.
17. Collecting data for audit purposes.
18. Compiling and issuing computer-generatedacute and repeat prescriptions.

Qualifications
Essential
31. Full GMC Registration with a licence to practise and entry on the GP Register - Certificate of completion date must be within 2 years and not having held a substantive post before.
32. Certificate of Completion of Training in General Practice / Certificate Confirming Eligibility for General Practice Registration, or equivalent.
33. Included on a Medical Performers List or eligible.
34. Evidence of ongoing professional development.
Experience
Essential
35. Experience of using electronic clinical patient records.
36. Experience of medicines management.
37. Experience of QOF and clinical audit.
Desirable
38. Understanding of policy developments related to the delivery of primary care services including General Practice, the GMS contract, Clinical Governance, Network Contract DES, and Quality & Outcomes Framework.
39. Knowledge of the health needs of the surgery population.

43. Experience with EMIS Web or similar clinical systems is an advantage.
