Job summary
An exciting Band 7, Advanced Practitioner opportunity has arisen to join a new Specialist Ventilatory Community Home Visits Service for an identified group of patients who, due to complex care needs and are at the end stages of their disease, are no longer able to attend Royal Papworth Hospital for specialist ventilatory review. The post holder will exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care, which may include highly complex respiratory presentations.
As an Advanced Practitioner, you will provide an autonomous, high quality, patient focused, efficient and comprehensive assessment of complex patients. The post holder will ensure they are working across the four pillars of advanced practice: Clinical, Leadership and Management, Education, and Research.
We are seeking an enthusiastic Advanced Practitioner who will help establish a five days a week community service for our RSSC patients. It is essential that you must be able to drive. If you are a highly motivated clinical expert and leader who can establish this service for our patients, then we are looking for you!
Main duties of the job
1. The post holder will conduct advanced clinical assessment of patients who have a wide variety of complex needs, including providing specialist advice on long term ventilation, airway management and sleep.
2. To develop and implement an individualised management plan, using advanced clinical reasoning skills and clinical expertise.
3. To effectively manage own clinical caseloads, and support others to manage their caseloads and prioritise appropriately.
4. To maintain accurate treatment documentation, according to department, Trust and registered professional body standards.
5. Attend the weekly multidisciplinary team meeting to discuss care and ensure decisions are communicated to patients in a timely manner.
6. To autonomously assimilate and interpret complex information to reach clinical decisions on referring patients on to other specialities.
7. To act as an independent prescriber (if the NMP course has been passed and the individual is listed on the Trust's NMP practitioners).
8. To provide expert advice, clinical support and training to all Respiratory Sleep and Support Centre staff.
9. To lead and support others with relevant audit, service improvement projects, and research with support from colleagues and proactive engagement of relevant other departments.
10. To lead service development/improvement/guidelines of care within the Trust.

Qualifications
Essential
* Professional Registration with either NMC, GPhC or HCPC
* Advanced Clinical Skills qualification
* Non-Medical Prescribing if appropriate to registered profession / role.
* Relevant role specific qualifications (i.e. Advanced Resuscitation -ALS/EPLS/ATLS or surgical skills qualification)
* Willingness to work towards a master's degree
Desirable
* Possession of master's degree in Advance Clinical Practice or equivalent / other MSc mapped to the four pillars of advanced practice.
* Leadership/management course or validated portfolio evidence (if not part of MSc level qualification)
Experience
Essential
* Significant experience of working with respiratory patients as a registered practitioner in a provider organisation (e.g. acute sector, community provider, third sector, social care or primary care) and within a relevant specialty.
* Proven experience of working with patients requiring invasive and non-invasive ventilation.
* Proven experience in the management of patients with primary sleep disorders such as Obstructive Sleep Apnoea.
* Proven experience of undertaking audits, Serious Untoward Incidences and change management skills.
* Experience as a Line Manager. Demonstrable evidence of mentoring, training/ coaching in staff development, supervision and teaching.
* Experience of collaborative working with the multi-disciplinary teams.
* Ability to work autonomously.
* Evidence of theoretical and practical skills allowing autonomous patient management (i.e. patient assessment, diagnosis and management) related to clinical scope of practice.
* Understand, encourage and support clinical supervision and mentorship both within and across professional roles.
* Effective written and verbal communication skills.
* Educational skills such as identification of learning needs, delivery and evaluation, providing work based learning.
* Experience of multi-professional working.
* Ability to interpret and evaluate national and international guidelines and apply to local protocols and practice as appropriate.
* Ability to communicate effectively with all stakeholders including to service users, clinicians, and senior leadership teams. This will require proven ability and leadership skills demonstrating motivational, visionary, diplomacy and negotiation skills.
* Works under own initiative with strong organisational skills and ability to achieve deadlines.
* Able to use high-level problem-solving and analytical skills and manage complexity.
* Effective time-management and prioritisation skills.
* Appropriate level of I.T literacy (i.e., for radiology, pathology, microbiology ordering as per Trust protocols, utilising patient systems).
Desirable
* Experience in physiotherapeutic management including cough augmentation.
* Experience of collaboratively working with other specialists supporting & developing programmes of care.
* Evidence of involvement in change management and/or clinical service improvement.
* Experience of writing and implementing Standard Operating Procedures in own area of practice.
* Research conversant with published research articles and peer reviewed materials.
