We are seeking a healthcare support worker to join us in providing care and treatment to adults living in the Fareham and Portchester area who have long term or complex health needs. The scope of your role will focus on supporting admission avoidance, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi-disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards. Our service runs from 0700-1700, 7 days a week and therefore this position requires flexible working and all requests will be considered.
