The Role:
Joining our Reablement Team as a Community Reablement Assistant Level 2, you’ll help to support adults over the age of 18 across Hampshire who may be experiencing a need for additional care and support. Our aim is to maximise their independence and potential, as well as reduce, avoid or delay the need for ongoing care whilst supporting them to remain living at home.
Our team of Community Reablement Assistants, Occupational Therapists and Sensory Officers, work in partnership with Health colleagues to offer support, encouragement and guidance to those with short or long term conditions and frailties as well as supporting those coming out of hospital.
What you’ll do:
* Drive to service users' homes and working independently carry out reablement intervention
* Work towards enabling service users’ independence in daily living tasks and personal care
* Deliver hands on care
* Complete working supervisions with Community Reablement Assistants Level 1’s and support their development
* Write up medication
* Undertake risk assessments
* Complete and review service users’ goals
* Use a variety of IT systems such as MS Teams and email

* Experience of working within a social or health care setting
* Effective communication skills both verbal and written
* Sound literacy and numerical skills
* Sound IT skills
* Motivated and work on own initiative
* Good observation, listening and feedback skills with the ability to seek advice from other appropriate professionals
* Good team player
* Able to prioritise delegated workload and demonstrate effective time management skills
* Leadership skills
* Willingness to be inducted and trained in enabling skills

Working Patterns
Our Service runs 07:00-22:00, 7 days a week, 365 days of the year, therefore you’ll be required to work evenings and/or public holidays and/or weekends as part of your Normal Working Hours for which enhancements are paid in line with the HCC salary policy. Details of the working patterns available will be discussed in more detail during the selection process.

Travel Requirements
