We aim to work in partnership with commissioners and others to develop and deliver high-quality care in the community to vulnerable people. Our goal is to provide housing with care based on an enabling approach with an outcome focused support planning process. As a large provider of social care services, Mears works collaboratively with multi-disciplinary teams to address challenges in the social care marketplace. About the Role: As a Care Coordinator you will be an exceptional leader, the mark of your success is to make a positive difference to people's lives. You will spend each day assisting in the day-to-day operation of the Support Services, business and participate as required in multi-disciplinary assessment of service user needs. The successful Care Co-Ordinator will be responsible for ensuring all care plans are reviewed on a 6 monthly basis, spot checks are completed on a 3 monthly basis, staff are reviewed quarterly, and staff annual appraisals are completed. This is an office-based role in Alva and you will be required to commute to services in and around Edinburgh and West Lothian. You will be able to claim millage when out completing any reviews at 25p per mile, from office to service user and back to the office. We are Mears Supported Living Ltd, providing a range of care and support services to people out in the community and in their own homes. Role Criteria: Direct line management for a team of Support Workers, including the management of their weekly workload Defining special care needs based upon identification of specific clinical problems and to monitor and review as appropriate Liaising with external organisations in respect of developing a total package of care for a service users Reporting to a Branch Manager on a weekly basis on activity levels and outcomes for service users Providing written reports of all complaints, accidents, incidents and untoward events in accordance with written policies Undertaking “on call” duties as required by the branch to ensure a 24/7 service is provided. This will be on a rota basis 1 week at a time 1 out of every 3 weeks. Experience working within the care sector and holding a completed SVQ 3 is essential for this role. Experience as a Care Co-Ordinator would be preferred, it would also be strongly desired to have excellent IT skills and competency in Microsoft Office.
