Job Overview
An Urgent Care Practitioner will be an experienced Allied Health Professional (AHP) or Nurse who will provide assessment and treatment of patients during a crisis. They will be a senior clinician within the team, responsible for identifying complex needs, and will develop, implement, evaluate programmes of care under the direction of Medical and ACP leads. The post holder will demonstrate safe clinical decision-making, as well as critical thinking and expert care. They will work collaboratively with other clinicians and organisations to meet the needs of patients and their families, supporting the delivery of policies and procedures, providing leadership when required. They will support junior colleagues in development, and delivery of safe patient care, and oversee clinical decisions as required to support the Doctor or ACP leads.
Main Duties of the Job
* To work alongside pre-existing Urgent Care Response (UCR) and Planned Care workstreams to deliver a seamless transition through services, delivering a see, treat and review model of care to evaluate patients' response to health care provision and review the efficacy of input.
* Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly, including patients who present with complex needs.
* Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan with support from Doctor and ACP leads.
* Prescribe or recommend initial medication, appropriate to patient needs and in accordance with evidence-based practice in line with local, regional, and national practice protocols and formularies.
* Work with patients and their carers to support concordance with agreed individualised treatment plans.
* Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects, and potential interactions.

Person Specification
Training & Qualifications
* Current registration with NMC or HCPC without restrictions on practice.
* Evidence of post-registration, Masters Level (L7) qualification appropriate to the post and in line with the pillars of advanced practice.
* Non-medical prescriber (V300)
* Demonstrable clinical supervision training or management experience or qualification.
Experience
* Demonstrable recent Urgent Community Response, Acute or Emergency Care experience.
* Management of minor illness, minor ailments, and injuries.
* Experience of compiling protocols and clinical guidelines.
* Experience of supervision or management of junior staff.
Knowledge & Skills
* Evidence of advanced clinical practice skills including advanced physical assessment, cannulation, phlebotomy, and interpretation of investigation results.
* Management of patients with long-term conditions, multi-morbidity, complex needs, including mental health.
* Knowledge of budgeting and appreciation for management of team finances in a responsible manner, i.e., equipment purchases and staffing recommendations.
* Clinical governance issues in community-based care.
* Excellent interpersonal and leadership skills.

* Must be able to meet travel requirements of the post.
