Job summary We are looking for a community Occupational Therapist with relevant experience to join this developing community service which provides discharge facilitation and admission prevention services in the Borough of Kingston. This particular post is primarily based with our integrated community OT team based at Hollyfield House, Surbiton, Surrey. The post is part of a team of community Occupational Therapists who are part of the broader multi-disciplinary Your Healthcare at Home service providing assessment and rehabilitation programmes to patients coming out of acute hospital care or needing support to stay in their community residence. The OT team work as an integrated team covering caseloads which currently include: Community In-patient beds ReAblement / Intermediate Care Falls Prevention Duty Occupational Therapist (7 day rota) Long Covid Secondary Care Hospital in-reach (including liaison with the SDEC project) PLUS links to our Urgent Response Teams and Virtual Ward. The post offers the potential for a wide variety of experiences, depending on skills and interests. The Occupational Therapy team also have links with OT colleagues working across the Borough in other organisations including Social Services, Acute Hospital and Mental Health. Work is on-going within Kingston to increase links between the OT services in the Borough as part of the exciting developments in the Kingston Co-ordinated Care Project. The post holder would have a clinical grounding in general community working with a caseload predominantly from the care of the elderly population. You would need to be organised, flexible and highly motivated to work with our service to deliver best possible outcomes for our patients.
