Job summary Join our team transforming community healthcare and mental health services As a Transformation Manager, you will play a key role in shaping and delivering clinically appropriate and sustainable services in Ealing, Hounslow and Hammersmith & Fulham. Supporting the senior transformation manager, you will be working on given transformation assignments to transform pathways, service models and deliver strategic priorities. Working in collaboration with senior clinical and operational leads the post holder will be responsible for end to end management of transformation initiatives providing expertise, leadership and robust processes to deliver improved outcomes and experiences for patients and carers. This includes, supporting activities, from co-production with stakeholders to ensuring estates, digital and other enabling solutions are established for all initiatives. While not a clinical position, the job holder will manage projects that impact clinical services, necessitating effective communication with staff at all levels, partner organisations and local system leads. (max. 300 words) Main duties of the job This job holder will manage key transformation workstreams across Local and Specialist Services CSU related to aligned with national policy frameworks (inc. NHS Long Term Plan) and local drivers of change at Trust, Borough Based Partnership and NW London ICB level You will play a key role in ensuring robust and effective change management processes are delivered. They will be responsible for end to end management of assigned projects including starting, maintaining, supporting, facilitating, monitoring progress, closing and evaluating multiple workstreams. The job holder will provide expert project management input as appropriate across the transformation portfolio. The job holder will take responsibility for all enabling work to drive the projects including co-production with service users, carers, staff and co-production partners; enabling digital solutions; enabling estates solutions; tracking outcomes; and optimising workforce. The job holder will work closely with colleagues across the Local and Specialist Services CSU including senior management within in service area, clinical leads and support services. They will also work closely with borough leads as well as NWL ICB Programme colleagues ensuring they are fully sighted and involved in all transformation work.
