Job summary Are you an enthusiastic support worker? The Home Treatment team in West Dorset are looking for a Support Time Recovery Worker to join As a support time recovery worker, you will offer support to a range of clients experiencing acute mental health difficulties and carry out assigned tasks as identified by the registered nursing staff in relation to patient needs. You will give time to individuals to promote their recovery and assist them to remain in the least restrictive environment whilst they recover. Driving is an essential as you will be visiting patients in their own home in locations across the county of Dorset to support their recovery in line with an agreed care plan and prevent hospital admission. You will work in a supportive multi-disciplinary team and have the opportunity to develop your skills and progress through training and supervision. This is a full time post, part time will be considered. The service hours cover 9:00 - 22:00 seven days a week with shift patterns of 9am-5pm or 2pm-10pm, condensed hours would also be considered. Main duties of the job You will work collaboratively with the inpatient staff to support service users in their discharge. You will be an active member of the multi-disciplinary team in assisting and engaging patients in organised, planned meaningful activities for patients. Help monitor the individual's progress, level of functioning, and mental state and alert the appropriate staff involved in their care. Monitor that service users are engaging effectively with the agreed recovery plan and access appropriate services provided on a regular and consistent basis and bring concerns to the attention of the Team Leader.
