Job summary Unplanned Service - Advanced Practitioner (Band-8a) Are you an Advanced Practitioner looking for an opportunity to work within a dynamic community team? The role includes providing enhanced nursing, and social and clinical assessment of housebound patients with escalating complex needs, within a robust MDT structure. BDCT covers Bradford and Airedale localities. The service runs seven-days-a-week, between the hours of 08.00 - 18.00. The primary function of the role is to maximise the patient's health, clinically assess and treat patients in defined groups thereby reducing unnecessary admissions to acute services, facilitating the delivery of effective, co-ordinated, and timely high-quality care to patients. Work collaboratively in providing flexible, dynamic and innovative care, responsive to unplanned health needs, focusing on those service users who present with complex needs, co-morbidities and long-term conditions. To proactively manage a time limited episode of care of very high intensity users of services, for treatment and onward referral to enable the patient to maintain independence, avoid unnecessary hospital admission and facilitate early discharge in coloration with Virtual ward pathways.
