Summary:
The Social Worker is primarily responsible for direct services that include psychosocial assessments, casework services, counseling, discharge planning, information and referral, group services, multi-disciplinary team conferences, case management, education, community/agency collaborations and/or visits. This position includes participation in department and hospital committees that may involve planning and development of programs and services; participation and planning in Continuous Quality Improvement activities as appropriate. The Social Worker may assist with the orientation and education of new employees and Social Work students when requested, as well as carry out other duties as assigned by the Assistant Vice President of Home Care.
The person in this position has the responsibility for planning and implementing in-service education programs that reflect Social Work standards professionally as requested. Assumes responsibility for self and SW assistants meeting Social Work standards and meeting requirements of federal, state and regulatory bodies. This Social Worker will assist with group facilitation as needed in areas of Ethics, Staff Management and/or Patient Care Management.
Personal characteristics include: imagination, creativity, cooperation, flexibility and resourcefulness. He/she must have good judgment, problem-solving skills, willingness to work as a team member, and ability to work independently using sound clinical Social Work skills with a variety of professional and/or disciplines. Courtesy, empathy, and friendliness to patients, families and hospital staff must be demonstrated.
This position will demonstrate competency in the knowledge of human growth and development ranging from: Neonate, Infant, Pediatric, Adolescent, Adult, Geriatric patient population and demonstrate sensitivity to patient’s cultural background.
This position may be required to cross train within the agency. Responsibilities in secondary position may require access to medical records and other computer access exceeding routine computer usage. Access to patient information may be necessary to fulfill the requirements of this role; therefore, adherence to Ballad Health privacy and security policies and practices is a must.
Requirements:
Must have a Master’s Degree in Social Work from an accredited School of Social Work. Experience in Medical Social Work preferred. Experience in Clinical Social Work desired. Activity in a professional Social Work association encouraged. Excellent public relations and organizational skills are necessary, as well as problem-solving and communication expertise. Participates in professional continuing education requirements as required by the appropriate state's licensure privileges.
