The Discharge Lounge are recruiting for Healthcare Support Worker to join our team working alongside the Discharge Lounge to facilitate and coordinate patient flow through the acute setting to the Discharge lounge.
You will provide and deliver a high standard of nursing care recognising and meeting the specific needs of the patients/clients. This could include undertaking duties as per patient care plan e.g. hygiene duties, providing 1:1 support to a patient.
In collaboration with the ward MDT to pro-actively support and plan patient flow throughout the hospital. The role requires a consistent and coordinated approach to the operational management of patient flow, with an emphasis on maintaining an on-going and ‘real time’ understanding of the ward’s current and predicted capacity position and the timely and well-planned discharge process.
* As requested, attend SAFER ward round and contribute, prioritise and identify tasks to support patient flow.
* Present findings to the patient flow Manager in real time, to include any escalations and report bottlenecks or delays.
* Update and manage the maintenance of swift plus to ensure it reflects the current discharge plans and the ward bed availability.
* Collect/deliver TTO (to take out) medications.
* To support the effective and timely flow of patients on the ward, support plans for admission or discharge.
* With support of MDT identify patients for Discharge Lounge.
* Facilitate transfer of patients from wards/ED/Assessment area (PULL model).
