Job summary NHS Buckinghamshire, Oxfordshire, and Berkshire West Integrated Care Board is looking for an experienced and dynamic Complex Case and Discharge Manager Learning Disability and Autism Inpatients to join its team. As a Complex Case and Discharge Manager Learning Disability and Autism Inpatient the post holder will be highly organised and motivated, with excellent communications and a track record of effective delivery in a complex environment. As part of the ICB Team the successful candidate will support the acceleration of our priorities, driving a culture of continuous improvement and collaboration working closely with other ICB teams and our system partners. Main duties of the job The post holder will be an experienced, dynamic, innovative Complex Case and Discharge Manager, responsible and accountable for supporting people through discharge out of hospitals, completing 8 weeks Commissioner Visits and represent the ICB at the Care & Treatment Reviews and provide information to support Commissioners make decisions. The post holder will support and advise Commissioners on a range of issues relating to hospitalised Adults with Learning Disabilities and Autism behaviours that challenge in relation to their clinical and social care needs. This role requires the Complex Case and Discharge Manager to support the ICB promote alternatives to admission, respond to emergencies, hasten discharge and facilitate oversight on quality of ongoing hospital care. The post holder will work closely with the Head of Sec 117 to reduce the number of people going into hospital, improve their quality of life and deliver alternative to admissions through networking with the community teams. Be accountable for producing patient reviews in hospitals (through Commissioner Oversight Visits) and reporting these into NHS England and the Berkshire West Mental Health and Learning Disabilities Integrated Care Programme.
