Admin & Prescription Clerk (PCN Care Coordinator)
The purpose of this dual role is to work as a Care Coordinator/Admin Assistant within the Primary Care Network and within a team of Prescribing Clerks located within the Pharmacy Hub at Brooklea Health Centre. This role supports Pharmacy Technicians, Clinical Pharmacists, and GPs in managing all patient prescription requests for the five constituent practices within Connexus PCN: Birchwood Medical Practice (Brislington), Nightingale Valley Practice (Brislington), Priory Surgery (Knowle), Stockwood Medical Centre (Stockwood), and Wells Road Surgery (Totterdown).

The role of the Care Coordinator/Admin Assistant in the Primary Care Network is to support our multi-disciplinary clinical team to ensure that our patients, particularly those who are vulnerable or have complex needs, are able to access services that meet their needs.
About Us
Connexus Primary Care Network brings together five GP Practices serving 50,000 patients in the Brislington, St Annes, Knowle, Totterdown, and Stockwood areas of South Bristol. The PCN is located in the South Bristol locality of BNSSG and employs a small staff team responsible for delivering a range of services under contract with NHS England.

Key Responsibilities and Tasks
To Support the PCN Prescription Hub
Full training will be provided for this role, and previous experience as a Prescriptions Clerk is not essential.
1. Process repeat prescriptions daily, ensuring compliance with the prescription hub protocols.
2. Perform regular searches on drugs requiring monitoring to minimize the risk of adverse effects.
3. Ensure all current medications have relevant, current authorizations.
4. Promote cost-effective prescribing at the practice through switching drugs to generic preparations when appropriate and encouraging the use of local primary care trust formulary medication choices.
5. Handle prescription queries from a range of healthcare professionals including GPs, nurses, community pharmacists, nursing home staff, as well as reception/administration staff at the practice.
6. Manage medication changes from all hospital letters, removing old medication, adding new or making changes, and adding any relevant monitoring diaries.
7. Perform searches and act upon drug alerts.
8. Add new patient medication and pass to registered GP for approval, highlighting any concerns found and adding relevant diary entries/recall. Liaise with patients via phone call, SMS, or letter, and with patients' pharmacy regarding changes if necessary.
9. Liaise with pharmacies regarding monthly dosette box prescriptions.
10. Be the first point of contact to coordinate responses to email/telephone inquiries.
11. Support the PCN team in coordinating audits to support improved patient outcomes.

Salary: £24,000 to £26,500 a year depending on experience.
