TEES COMMUNITY NURSING TEAMS (East Cleveland PCN)
An exciting opportunity has arisen for registered nurses to apply for a community staff nurse which will be attached to a Primary Care Network (PCN), to provide coordinated care through integrated teams.
Primary Care Networks signal a significant change in the way practices interact not only with each other, but the wider health and social care system. This will enable the traditional barriers to the delivery of health and care to be broken down to enable care to be delivered to a defined patient population, in a different way to meet the needs of that population, focused on prevention and personalisation of care, making best use of resources collectively.
The NHS Long Term Plan introduced PCNs as essential building blocks of every Integrated Care System. The postholder will have responsibility for leading and managing the community nursing workforce.
The community staff nurse will be attached to a caseload of patients where responsibility lies with the District Nursing Sister within a PCN. This role will provide the opportunity to work closely with GP practices and other partners, in supporting patients with highly complex and often high acuity needs to prevent unnecessary hospital admission. This role requires a clinician with excellent team working and communication skills, to provide patient centred care and deliver effective evidence-based care to a defined demographic population with diverse health care and social needs.
The successful candidate needs to be prepared to adapt to the changing needs of the Trust and the community models of care as they arise.

You will be expected to attend the leadership development programme and the Quality Improvement Programme that the Trust delivers. This programme aims to:

* Develop your Leadership effectiveness and skills
* Practitioner level in quality improvement, equipping you with the skills to champion, lead and complete quality improvement within your role
After you complete this four and a half days of training you will be able to explore further leadership and improvement training opportunities; we offer further in-house courses dependent upon your role and bitesize programmes and leadership apprenticeships at level 3, 5 and 7.
