Job Overview
Knowsley community therapy team is seeking a committed Band 7 Clinical Lead Physiotherapist to join our service supporting the patients and staff through shared care within their localities. The post holder will provide expertise and advice in the treatment of highly complex conditions and mentorship to those within the community therapy team. They will have continuing responsibility for the management of a defined caseload and a coordination role of the daily activities.
We are seeking a highly skilled, enthusiastic, and proactive therapist to work in collaboration with the service lead to support service modernisation and redesign, with the service responding to the rehabilitation needs of adults 18+, who have multiple complex co-morbidities or support with discharge from the hospital setting.
This is a fantastic opportunity to join our multi-disciplinary team, which consists of Physiotherapists, Occupational Therapists, Assistant Practitioners, and Therapy Assistants. We also provide Physiotherapy intervention to other teams within our portfolio, specifically the Falls team, Rydal ward, and the Physical health in reach (PHIR - mental health). The OT and Physio support input within the Willowbrook hospice and guide patients' home from the short term placements beds within the community.

Main Duties of the Job
The post holder will provide clinical leadership for staff, through supervision and PDR. They will participate in planning, development, and evaluation of the services within the designated area/team, ensuring the delivery of the quality strategy incorporating establishment of systems and processes.
The post holder will be expected to effectively manage a caseload of patient's comorbidities as an autonomous practitioner, using evidence-based/patient-centred principles to assess, plan, implement, and evaluate interventions of the patient group. Maintain clinical credibility by providing clinical care and supporting members of the team within the clinical environment. Acts as an advocate and champion for patients/clients in a variety of forums and professional groups and challenge attitudes and behaviour. Work in partnership with other professionals to enable service users/patients to be maintained in an appropriate environment, thus avoiding hospital admission.
To contribute to the maintenance and development of the Physiotherapy profession within community therapy and will facilitate effective learning within the area of practice for all staff, students, and practitioners.

Detailed Job Description and Main Responsibilities
1. Provide clinical leadership and management to the clinical team.
2. Be responsible for the delivery of the service within the area.
3. Work in partnership with other services/stakeholders etc., to enable patients/clients to be treated in the appropriate setting.
4. Ensure that the team provides a high-quality service to its clients by providing caseload and clinical supervision.

6. Ensure that teams work proactively in order to deliver anticipatory and maintenance care, providing a responsive service to patients/clients with both planned and unplanned care needs.
