Job summary An exciting opportunity has arisen for experienced and enthusiastic pharmacists to join our friendly and forward-thinking team based within the Royal Primary Care Chesterfield / North Derbyshire PCN. We are seeking pharmacists who are highly motivated, innovative, and keen to develop clinical skills as part of the multi-disciplinary team. Applications from pharmacists with experience in primary care are welcomed especially form those who have completed the CPPE pathway training (although this is not essential). This role is purely base in primary care but as a part of Chesterfield Royal Hospital, RPC is uniquely placed to develop strong links between primary and secondary care pharmacy roles. Main duties of the job You will undertake a clinical pharmacy role within North Derbyshire PCN practices. This will involve a wide variety of tasks that will allow enthusiastic individuals the opportunity to develop their skills. The role will involve independent prescribing and as a PCN role structured medication reviews. You will also be expected to undertake a senor role within the team in the provision of medicines governance advice or education and training needs. public health and social needs of patients within ND PCN member practices Provide expertise in clinical Structured Medication Reviews (SMR's) and address the public health and social needs of patients within ND PCN member practices Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need following discharge Contribute to reductions in medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicines related
