Key responsibilities include conducting thorough assessments, developing care plans, and collaborating with services to ensure seamless access to mental health support. This role requires a person-centered, recovery-focused approach and compliance with statutory duties under the Care Act 2014 and the Mental Health Act 1983.
* Conduct assessments to identify mental health needs.
* Carry out mental capacity assessments and make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate.
* Respond to emergencies and crises, providing immediate support and intervention.
* Work with a multidisciplinary team to coordinate comprehensive care.
* Maintain accurate records of assessments, interventions, and care plans.
Requirements: Relevant social work qualification, 2 years experience in adult social work, including assessment and care planning.
