Job Description
Main Duties & Responsibilities
* Register with Social Care Wales within 6 months of commencing employment, including maintaining your registration with CPD and associated annual costs.
* Act in accordance with the standards set within the Social Care Wales Guidelines and requirements.
* Maintain a professional manner at all times, reflecting the high standards of Liberty Care.
* Hold the appropriate qualifications as defined by the qualification framework for social care and regulated childcare (Wales), currently a QCF Level 2 or above, or be willing to work towards this.
* Support the personalised development of each individual you support, recognising their talents, sensitivities, and beliefs.
* Respect the dignity and individuality of everyone we support, providing them with choices where appropriate, and uphold non-discriminatory practices.
* Assist in the day-to-day running of their home and in all matters regarding the individuals’ daily lives.
* Ensure high standards of care and support are maintained, adhering to all requirements and applicable regulations such as the Social Services and Wellbeing (Wales) Act 2014 and the Regulation and Inspection of Social Care (Wales) Act 2016.
* Work as part of a staff team in a constructive and supportive manner, attending regular staff meetings, seminars, and individual support and supervision meetings as necessary.
* Work within a rota’d shift system inclusive of evening and weekend work (including public holidays) and sleep-in when arranged at a predetermined payment.
* Be ‘on-call’ & “Standby” by arrangement as per contract.
* Undertake tasks as delegated by the line manager.
* Ensure the safety of the individuals at all times, reporting promptly any concerns about their health, safety, or wellbeing.
* Establish positive and trustworthy relationships with individuals and relatives while adhering to the Code of Professional Conduct and Duty of Candour.
Individual Support & Key Worker Responsibilities
* Support the personalised development of each individual, recognising their talents, sensitivities, and beliefs.
* Respect the dignity and individuality of each individual and provide them with choices where appropriate.
* Ensure a high standard of personal care for each individual, including matters of physical well-being and personal hygiene.
* Initiate and support individuals to be involved in educational, occupational, and recreational activities when appropriate.
* Be flexible and support using a variety of communication systems to uphold individuals' needs, wishes, and requests.
* Be aware of the emotional needs and aspirations of each individual and provide opportunities for appropriate emotional expression.
* Contribute to the overall cleaning of the individuals’ home and involve the individuals in these tasks as appropriate.
* Write and keep updated individual Care Plans and be actively involved in the review process.
* Follow all agreed Care Plans, Risk Assessments, and individual guidelines, reporting any progress or problems.
* Report promptly any concerns about health, safety, or wellbeing of individuals.
* Assist individuals in maintaining good contact with family and friends.
* Support individuals to go on an annual holiday in line with their wishes.
* Act as key worker to individuals as agreed with the Manager.
* Complete keyworker reviews/monthly commissioning reports as required.
General Responsibilities
* Compile detailed written records in the individuals’ daily files, noting and reporting any changes in their health and behaviour.
* Administer medication in line with all Policies and Procedures and maintain all associated records.
* Accurately maintain all records as required by the Domiciliary Manager.
* Have a working knowledge of all Policies, Procedures, and guidelines within Liberty Care.
* Adhere to the Social Care Wales Code of Conduct, available in the Induction Handbook.
* Undertake courses and pursue professional development and training as encouraged by the Company.
* Be aware of all contact details and telephone numbers that may be required in an emergency.
* Notify the manager of any new criminal convictions, including motoring offences, and maintain annual subscription to the Disclosure and Barring Service.
* Provide evidence of driving qualifications and comply with 6-month driving licence checks (if applicable).
* Notify the Manager of any changes in health that may affect your role.
* Be flexible in terms of work base to meet the needs of the service, working across the company as required.
* Report any Health & Safety/Maintenance concerns to the Team Leader/Head Office and comply with all training provided by Liberty Care related to Health and Safety.
