Looking for an individual with experience in acting as an NHS Digital and Transformation Lead. The role is for 6 months and we are looking at 8 hours per week to assist Pelham Medical Practice with the following projects:
Aid in the design and implementing a new business model which will allow the GP practice to move away from being demand driven to a needs driven service and to help us utilise our capacity as efficiently and appropriately as possible.
Help identify providers of a total triaging system which will allow Pelham to meet its aim of moving towards supplying a needs driven service to its patients. Working within the following scope and restrictions:
1. Implement a triaging system which will not materially reduce the number of appointments we currently offer as a practice.
2. Due to financial constraints and full utilisation of clinical rooms, we cannot have a system that will require us to hire further clinicians to support daily triaging of online consultations.
3. Implement an online triaging system that is available to patients 24/7 and to automatically offer appropriate appointments for patients to book. This we hope will reduce the 8 am rush for appointments.
4. The new online triaging system should minimise digital exclusion.
Main duties of the job
Support the management team with the implementation of the chosen triaging system.
Lead in the development and implementation of a new EMIS Web appointment book template which will interface with the new triaging system so it can offer patients the ability to self-book appointments across both our surgeries with an appropriate clinician and in a timely manner as per the triaging recommendation.
Assist in the selection and development of a new practice website that meets NHS requirements.
Be responsible for the setup of the new practice website, managing the transfer of relevant information from the legacy website. Build in enhancements like staff photos, news updates, local health promotions etc and links to the NHS App, Joy App. Train practice staff in managing and updating the new website.
Help in selecting and the installation of new patient call and information screens and help support staff with training and how to maintain the information and news feeds.
Aid the practice in deciding if a self-check-in system is required at both surgeries.
Investigate document workflow (Docman) systems and processes current and new solutions to see what efficiencies and benefits could be secured by the practice and at what additional cost.

Pelham Medical Practice is a growing GP Practice with over 15,250 patients working across 2 GP Surgeries within Gravesend, Kent.
The Practice currently has an application to build a new purpose-built surgery but needs to make changes to the current way we work to allow us to continue growing our patient list and to ensure the level of service we deliver to our patients does not suffer before we move into our new expanded facilities.
This is why we are looking for an IT and Digital Transformation professional to help us implement a new business model.

HOURS: 8 hours per week, 6 Month Fixed Contract
Job Summary:
Work with clinical and operational teams within Pelham Medical Practice to create a vision to guide service improvements, identify opportunities to make tangible positive transformation, and provide the knowledge, expertise, resource, and tools to deliver changes in a collaborative way.
Support Pelham Medical Practice with their vision and help to develop it in alignment with NHS England and Kent and Medway ICB strategies.
Plan, coordinate, and directly support delivery of transformation activity and improvement projects in Pelham Medical Practice with a view to improving patient access, staff and patient satisfaction, and the efficiency and sustainability of general practice services.
Work closely with staff working at Pelham Medical Practice to understand current service models, identify challenges, agree priorities for improvement, and then support these to be delivered.
Support the planning and delivery of improvements that will provide efficiencies and benefits to patients and staff.
Ensuring any improvements will not digitally exclude patients from using the services provided by Pelham Medical Practice.
Develop relationships with system providers, Kent and Medway IT Leads, and SCW CSU IT Support Teams.
Ensure all systems link and interface to secure that all system benefits are captured and that all functions are fully utilised to implement an integrated solution which is easy to access and use for both patients and staff.
Develop robust testing and test scripts for any new developments to ensure a successful go-live and implementation.
Help in promoting and communicating all improvements to staff and patients, explaining reasons for making the changes and benefits to patients.
Prepare and deliver training to staff and to patients to ensure a smooth implementation of any improvements.

Measures should be taken to ensure access to the system is inclusive and patients directed to resources and agencies (like Libraries, Digital Kent etc) that can help them with affordability, hardware access, capability issues, and digital skills.

Qualifications
* Experience of having worked in the NHS as a Digital Transformation Lead
* Qualified to degree level and ideally in an IT discipline
* Understanding of GP Practices and systems that they utilise
