The Mental Health Liaison service (MHLS) offers a comprehensive range of mental health specialties within one multi-disciplinary team so that all patients, regardless of age, with a suspected or actual mental health problem can be assessed, treated, signposted, or referred appropriately regardless of age, presenting complaint, time of presentation or severity.
An exciting opportunity has arisen for a Band 7 Senior Practitioner to join the team, leading on work with patients presenting with co-occurring mental health and substance use difficulties. The successful candidate will work as part of the MHLS leadership team and be an experienced, driven, and passionate individual, able to work collaboratively with both patients and wider agencies, to ensure that people with co-occurring conditions are able to access the right care to meet their individual needs. The successful candidate will have a proven track record in the area of co-occurring conditions, demonstrating comprehensive assessment, risk formulation and care planning skills.
Main duties of the job
* Provide clinical leadership, supervision, support, and consultation to nursing staff and MDT staff in the MHLS team, in line with relevant pathways as outlined within the operational procedures, with particular focus on clinical pathways for co-occurring conditions.
* Support patients with co-occurring conditions, both those presenting to the Emergency Department and for inpatients referred to the service from the general hospital wards, taking a lead clinical role in the completion of highly developed assessments, risk management, interventions and onward care planning for this patient cohort.
* Form excellent working relationships with, and work closely alongside, a range of stakeholders, external agencies and wider community-based mental health and substance misuse services.

Essential
* Registered professional with relevant professional registration (NMC, SWE, HCPC)
Desirable
* Non-Medical Prescribing
* Qualifications in teaching/training.
Experience

* Have a minimum of 3-years post qualifying experience.
* Have significant experience of working with individuals with co-occurring mental health and substance misuse conditions
* Have experience of working within urgent care services
* Have experience in the management of complex cases, relating to physical and mental health
* Have experience in a senior/leadership role, supervising junior members of staff
* Have a varied range of experience which may include dementia care, acute care, supporting people who self-harm and misuse substances

* Be competent in the application of Bio/psychosocial assessments across a varied patient illness profile including dementia, functional illnesses and self-harm.
* Be competent in the area of risk assessment and formulation of risk management plans for a range of complex presentations, particularly for those with co-occurring mental health and substance use conditions
* Be able to undertake complex formulation and discharge/care plans for a wide range of people including those with dementia, functional illnesses, self-harm and substance misuse
* Possess excellent verbal and written communication skills with a wide range of people including professionals, patients and carers
* Be able to liaise effectively and communicate complex information to a range of other disciplines, agencies, and carers.
* Be able to manage a caseload and work autonomously within the team, supporting junior staff with caseload/workload management
