An exciting opportunity has arisen to work primarily as part of the Community Mental Health team focusing on the direct needs of the service users. Participating in the delivery of individual treatment plans across boundaries of care, working closely with senior staff and Care Coordinators providing support and care according to PCN team Model and My Care and Support plans.
Main duties of the job
To provide support and give time to service users within Navigo who are difficult to engage and have complex needs. To provide holistic care, taking into account psychosocial and physical healthcare needs of the client group and to undertake rehabilitation activities alongside service users.
To assist the Care Coordinators or key and service users to assess, plan, implement and evaluate individual care plans.

Job responsibilities
The Post Holder must have the individual service users' needs at the fore at all times and use the agreed set of values and skills to underpin their day to day work.
To work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis.
Ensure that service users engage effectively with the agreed care plan and participate in community support on a regular and consistent basis.
Provide regular and practical support to service users and their carers/significant others in developing and managing dignity and independence.
Provide support and assistance, as necessary, with daily living activities.
Positively promote independent living skills of service users.
Work flexibly with Care Coordinators, other relevant community support, service users, and significant others to implement the plan of care.
Help service users gain access to resources, including benefits and welfare rights.
Help identify early signs of relapse by monitoring the service user's progress, level of functioning, and mental state and alert the appropriate staff involved in the person's care.
Assist the Care Coordinator and service user in developing crisis/contingency plans and advanced statements, as per local policy guidelines.
To maintain adequate records, as required, by existing procedures, entering appropriate details on the service user's electronic record and any paper held files.
To be able to communicate routine information in an arena where there will be barriers to understanding, and to be able to use tact and persuasion in a non-aggressive manner.
Attend and participate in agreed training, development, and supervision sessions as an individual and as part of the team.
To undertake such other duties as may be determined from time to time within the general scope of the post.

Person Specification
Qualifications
* Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience.
* A basic knowledge/understanding of the Mental Health Act.
