Whitby Group Practice is looking for a part-time Pharmacist to join our existing pharmacy team, which includes 2 pharmacists and 2 pharmacy technicians.
They will also be supported by a dispensary team with excellent knowledge and will provide support to deal with prescription and medication queries. This includes supporting the repeat prescription system, dealing with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice.
The post holder will undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The role is pivotal to improving the quality of care and operational efficiencies, so it requires motivation and a passion to deliver excellent service within general practice.
Main duties of the job
* Undertaking medication reviews with patients over the phone, face to face, and in their home or care home.
* Providing support to patients with medication queries and concerns.
* Answering relevant medicine-related enquiries from GPs, other practice staff, and other healthcare teams (e.g., community pharmacy).
* Reconciling medicines following discharge from hospitals, intermediate care, and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
* Ensuring that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time.
* Managing the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates, flagging up those needing a review.
* Implementing changes to medicines that result from MHRA alerts, product withdrawal, and other local and national guidance.

The Practice is located in Whitby and has a branch site in Robin Hoods Bay. We have around 13,500 patients.
Currently, there are 4 partners supported by a team of several salaried GPs, Advanced Care Practitioners, Nurse Prescribers, Practice Nurses, HCAs, Pharmacists, Pharmacy Technicians, a Paramedic, a dispensing team, care coordinators, and a committed admin and management team.
We are a training practice that works with Hull and York Medical School and currently supports Year 4 medical students, GP Registrars, and F2s.
We have been rated Good by CQC and are a consistently high QOF achieving practice.
Our clinicians have daily lunchtime meetings to discuss ongoing cases and ask questions of colleagues.
The practice is a very desirable place to work with a relaxed and pleasant atmosphere. It is very important to the partners that this is maintained and developed.
There is a good social aspect to the team, and we always have a Christmas party and a summer BBQ with several events in between.

Qualifications
* Mandatory registration with the General Pharmaceutical Council.
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
* Independent prescriber or working towards/intent of gaining independent prescribing qualification.
Experience
