We have an exciting opportunity for a Social Prescribing Co-ordinator to join our healthcare teams across our West Yorkshire team, working across HMP Leeds, HMP Wakefield, and HMP Wealstun. As a Social Prescribing Co-ordinator, you will be responsible for connecting people to activities and services within the prison, and required to support and motivate patients to access opportunities to help improve their health and wellbeing, whilst reducing dependency on medication.
This is a new initiative being brought in across our Yorkshire Region. All work will be based on site, and you will be expected to work at all sites as part of this role.
Hours: 9am-5pm Monday to Friday
Salary: This position is full time, and you will receive an annual salary up to £33,000 per annum depending on experience.

Interviews will take place on Friday 10th January at HMP Hatfield.
Main duties of the job
As a Social Prescribing Co-ordinator, your responsibilities will vary. You will:
* Complete initial assessments with patients referred to and eligible for the social prescribing service.
* Assess patients' current wellbeing and identification of what matters to them.
* Work with patients to develop personalised wellbeing plans based on services and activities that offer practical, social and emotional support that will improve their health and wellbeing.
* Manage a caseload of social prescribing patients providing follow-up support and ensuring they are able to access prescribed services with confidence.
* Be responsible for the scheduling and sequencing of patients' social prescribing activities.
* Liaise with Health and Prison departments, to develop awareness and support delivery of the service.
* Ensure effective cross-departmental and regional working in order to meet our aims and objectives of a whole system and asset-based approach to social prescribing.

Qualifications required
* A positive, inclusive and compassionate practitioner.
* Experience of using SystmOne.
* Experience offering an information, advice and guidance service in a health, care or similar setting.
* Experience of working within a prison environment or working closely with patients with challenging and complex needs.
* Experience of managing own caseload and understanding how to prioritise patient need and manage a complex caseload.
