Job summary Job Summary: The primary focus of the post is to ensure that all patients and families receive high quality patient centred care, reflecting the Trust objectives, values, and vision. At all times the practitioner will act as an advocate for patients and families and will be a front-line advocate for the Trust. They will adhere to and understand all Trust and local policies and procedures Main duties of the job Accurately assess, plan, implement, monitor, and evaluate the holistic pre and post -operative care needs of patients using an extensive knowledge base and range of skills. Deliver excellent standards of clinical practice to all patients and support colleagues to deliver the best possible outcomes and experience for patients. Act as patient's advocate and ensure patients' wishes, choices and beliefs are considered in the decision-making process as necessary in collaboration with the family. Using evidence based best practice maintain safe and effective patient care, ensuring that working practices conform to local and national policies and procedures Act as a clinical reference point and subject matter expert for designated area of responsibility. Ensure the WHO safety, checklist is completed for every theatre list and patient undergoing a surgical or anaesthetic procedure. Develop and maintain local and national standards and ensure practice is evidence based. Maintain accurate and legible records of care in line with Trust policy and NMC Code of Conduct. Be involved in monitoring and promoting high standards of care, including audit of quality indicators, and devising action plans to address any deficits identified. Maintain a culture in which infection control is intrinsic to care delivery and is monitored in line with Trust policy.
