This is an exciting opportunity to take a senior role in the West Essex Hospital at Home and Urgent Community Response Service.
The post holder will work with the senior management team to provide comprehensive, operational, staffing and budget management. They will be responsible for the efficient and effective day-to-day running of the Hospital at Home (HatH) and Urgent Community Response Service. This will include the delivery of operational performance targets (finance, activity, workforce and quality/safety) along with the achievement of service objectives and compliance with local and national standards relevant to the service area.
As a member of the leadership team, the post holder will share responsibility for the implementation of all Trust policies and the service improvement agenda within their area. As Matron, they will also provide clinical and professional guidance within areas of responsibility regarding operational issues. This will include being the Lead for Utilisation, Admission process, Discharge Planning and Length of Stay management. The post holder will also be expected to load on the investigation of complaints as directed by the senior management team.
• To be responsible for all aspects of operational management of the HatH service
• To ensure all patients receive safe and compassionate care and experience a great service
• To lead a team of multi-disciplinary clinical staff, ensuring resources are used to maximise the benefit to all patients
• Take a lead in developing specialist skills within the team to help reduce the demand on acute sector care
• Ensureclinicalcareisappropriatelymonitoredandauditedandlearningissharedacross theorganisation
• Lead daily MDTs and contribute to case conferences ensuring there is a holistic assessment of patients asrequired
• To set outcome based targets for the service and manage performance againstthese
• To respond to informal and formal complaints received in line with Trust policy; make changestoservices,systems,policies,practiceandcommunicationasappropriateinline withpolicies
• To be responsible for communicating highly complex information to staffinrelation to servicedevelopments.Thismayincludethecommunicationofdevelopmentsthatwillnot be received positively, for example changes to serviceprovision

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required.
