An exciting opportunity to be part of a dynamic team at the heart of hospital discharge has arisen.
The post is for a Band 5 Social Worker working within the Discharge to Assess team covering the South West area.
Multi-professional working is a key skill required as you will work alongside therapists, care providers, district nurses, users and carer's mental health services, and commissioners to support people to return home and regain their independence. Good communication skills are a necessity. The interventions are short term and the work is of a fast pace but rewarding when the outcome is that the person is able to live as they choose in their own home.
Main duties of the job
As a Band 5 Social Worker, you will undertake the more complex interventions, requiring good knowledge of The Mental Capacity Act and The Care Act to guide practice and ensure the best outcome for users and carers. A key responsibility is to complete timely person-centred, strengths-based assessments that ensure autonomy of the people we serve.
You will be required to support Band 4 and Band 3 staff with more complex cases.

Part of your role will be to contribute to work with Vulnerable Adults by completing safeguarding assessments, identifying, assessing, and managing risk to ensure the safety and well-being of all that access the service.

Social Work Learning Academy (SWLA)
On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework. The relevant SWLA stage (i.e., stage 1 (ASYE), 2 (Social Worker) or 3 (Advanced Social Worker) will also be agreed taking all prior Continuous Professional Development (CPD) and relevant experience into consideration. The post holder will be required to take responsibility for their own Continuous Professional Development.

Essential
* Professional Social Work qualification, DipSW, CQSW, CSS, BA, MA or Foreign qualification where certified by the Professional Social Work regulatory body
* Ensuring current registration with the Professional Social Work regulatory body at all times.
* An ongoing commitment to personal and professional development.
* To undergo appropriate training to achieve Assessed Year in Practice (ASYE)
* Good keyboard / IT skills including word processing, spreadsheets, email and PowerPoint
