Children and Young Persons Prevention & Outreach Worker Cwm Taf and Bridgend Drug and Alcohol Service Job Description Role Children and Young Persons Prevention & Outreach Worker Service Choices within Cwm Taf & BISMS contract Location: Flexibility to work across all areas of Cwm Taf and Bridgend, where appropriate is essential. Barod Base To be confirmed Hours: The Outreach and Prevention Workers will work 37 hours per week. The office bases are open Monday – Thursday 9am to 5 p.m. and Friday 9am to 4.30pm, but these posts will be required to work flexibly outside of these hours to meet the needs of children and young people. Some weekend working will also be required. Salary: Salary band £25,750 to £27,759 per annum Reports to: CYP Team Leader Team Members Reporting to Job Holder: No direct reports. Contract: In line with funding THE SERVICE Cwm Taf Drug and Alcohol service and Bridgend Drug and Alcohol Service provides a service to people who use drugs or alcohol, their families, and communities.This will include early intervention, education and awareness raising,engaging and assessing service users, needle exchange, brief interventions, outreach, training, case management, health promotion and harm reduction initiatives and recovery and aftercare services. ROLE PURPOSE The role of the Children and Young Person Prevention & Outreach Worker will be based within the Choices team.The primary function of this role is to work alongside other agencies to offer drug and alcohol support services in the community.This will include education, ensuring young people are aware of the risks associated with substance use.We will also ensure that young people in our community have early access to structured support and intervention. MAIN DUTIES AND RESPONSIBILITIES To use outreach to engage proactively with children and young people and potential high risk groups to allow early identification of substance use issues and prompt interventions as appropriate Provide targeted outreach services to individuals who are identified as ‘hard to reach’ to encourage engagement in the children’s and young person’s treatment services. To promote the single point of access entry point to children, young people and professionals. To work with children and young people in a way that promotes the Trauma Recovery Model Provide training or service talks to internal staff or external organisations as required To deliver awareness sessions, workshops, and educational events in various settings for children and young people Inspire and enthuse children and young people to maintain positive lifestyle changes Develop strong relationships with partnering agencies and other workers in order to enhance children and young people’s support mechanisms Ensure appropriate safeguarding and data protection principles are adhered to in working practice Deliver mobile needle exchangeservices where relevant Deliver mobile C-Card services Promote outreach campaigns Deliver harm reduction initiatives including Naloxone and Nyxoid training, take home kits, and Dried Blood Spot testing where age appropriate. Use a range of evidence based age appropriate interventions and methods that have been agreed for use within the service for children and young people Conducting risk and comprehensive assessments in order to assess individual needs, identify goals, prepare a support package and to review outcomes on a regular basis Providing crisis intervention where necessary and appropriate Signposting and supporting access to young people to other relevant agencies Empowering individuals and enable them to help themselves through informed choice and decision making. Improving and supporting young people’s motivation for change. Supporting young people to maintain or build positive social networks. Signposting identified populations affected by drug and alcohol use to appropriate services where they fall outside of the remit of the specification. To ensure consultation and participation with young people in all aspects of service delivery Maintaining good working relationships with key partners across all Tiers in particular the young person’s drug and alcohol specialist services connected to S-CAMHS, Integrated Family Support Services, Social Services, Family Services, Youth Engagement Servicesand Youth Justice Services. Complete duties where appropriate providing needle exchange duties and telephone duties being first point of contact for referrals Record all relevant data onto the case management system/s, and use the systems effectively to inform work with children and young people who use substances or as concerned others. Take up Barod’s arrangements for support and supervision and participate in all relevant internal meetings To develop your own knowledge and practice by enrolling on internal and external training opportunities Communicate Barod’s vision and deliver its mission, corporate strategy, and business plans Co-operate with requests from management and administration To record beneficiary data and outcomes in accordance with Barod policy to ensure the integrity of information included in Quarterly and Annual Reports to commissioners. Essential Desirable Qualifications and training Expected to meet basic skills in numeracy and literacy. Level 3 Diploma in Health and Social Care (or equivalent or relevant professional qualification) or willing to work towards. Youth Work Qualification or equivalent/relevant or willing to work towards. Facilitator Qualifications or equivalent/relevant or willing to work towards.
