The Older Adult Home Treatment Team provides a service 8-8pm, 365 days a year to older adults experiencing a crisis in the context of functional mental health or dementia. We work intensively with service users in a community setting to prevent admission to a mental health ward, support discharge from a mental health ward and provide comprehensive assessment and treatment plans in the person's own home. You should be able to demonstrate your passion towards working with this client group and have the ability to assess, co-ordinate treatment and support clients and carers.
