Job summary
An exciting opportunity has arisen for a Full-Time position of a Community Psychiatric Nurse within the Older Persons Community Health Team based in Port Talbot.
We are looking for an enthusiastic RMN to join our friendly Multi-disciplinary team. You will require experience of working in a mental health setting and an awareness of the delivery of community mental health care.
You will be a registered Mental Health Nurse, required to demonstrate a high level of commitment, motivation and knowledge of working with people with severe and enduring mental health problems.
You will be able to implement specialist skills, knowledge and experience for those identified as having Mental Health needs.
Main duties of the job
The post advertised will enable the post holder to engage with patients that are living the community primarily over the age of 65 that are presenting with symptoms suggestive of an organic illness (dementia, cognitive impairment) and other functional illnesses including depression and anxiety. The Post holder will be autonomous and responsible for arranging assessments, implementing and evaluating care, including the formulation of risk assessments and ensuring involvement of the MDT. The post holder will work closely with the Consultant Psychiatrist, Social Services, as well as third sector and voluntary agencies to ensure the patients are receiving the right care and interventions that have been identified.

Qualifications and Knowledge
Essential
1. Professional Qualification -RMN
2. Experience in Community Nursing Older Persons
3. Able to demonstrate clinical expertise for band 6
4. Knowledge of clinical risk assessments
5. Knowledge of Safeguarding Adults/Children including the Mental Health Act and Mental Capacity Act and Deprivation of Liberty Safeguards
Desirable
6. Teaching qualification (or equivalent experience)
7. Recognised management qualification
8. Understanding of wider nursing and health care related agenda
