The Medical HSEP (Health Services for Elderly People) team, specialising in working with patients living with frailty, is excited to be able to offer a Band 6 Physiotherapy or Occupational Therapy post.
You will be joining a friendly, well established integrated therapy service, working alongside experienced colleagues and supported by experienced occupational therapists and physiotherapists as well as a Frailty Lead Therapist/Advanced Clinical Practice Trainee and Clinical Lead.
Person-centred care and a supportive approach for our patients living with frailty are at the centre of our philosophy.
Main duties of the job
You will need to be a dynamic team player with excellent communication skills and be able to demonstrate a broad range of experience. You will be able to demonstrate a high level of clinical reasoning, organisational and prioritisation skills in order to manage a caseload effectively.
Working for our organisation
You will be joining an experienced multidisciplinary therapy team of Physiotherapists, Occupational Therapists, Speech and Language Therapists, Dietitians and Therapy Support Workers, supported by a Clinical Lead Therapist and Frailty Lead Therapist/Advanced Clinical Practice Trainee.

Education & Qualifications
* HCPC Registration
* Diploma / Degree in Occupational Therapy or Physiotherapy
* Member of Special interest group eg. SAM, AGILE
* Evidence of attendance at relevant postgraduate courses
Experience
* Experience in specialist area at Band 5 level
* Experience with complex discharge planning
* Experience working with patients living with frailty
* Experience of working at band 6 level
* Able to display transdisciplinary working competencies across Occupational therapy and physiotherapy.
* Able to work under pressure.
* Able to document information accurately.
* Able to demonstrate sound reasoning skills.
* Effective Communication Skills.
* Ability to participate in weekend, bank-holiday rota.
* Evidence of teaching and supervision skills
* Evidence of student or physiotherapy assistant supervision
* Involvement in audit research and understanding of quality issues
Personal Qualities & Attributes
* Evidence of CPD maintained in a portfolio
* Demonstrate knowledge and understanding of current occupational therapy or physiotherapy practice in relevant area
* Awareness of evidence-based practice
* Understanding of legal responsibilities of the profession such as confidentiality, consent and documentation
* Understanding of clinical governance and its implication for services, including experience of quality issues and audit
* Demonstrates a desire to work in area of frailty
* Knowledge of current NHS and local issues
