This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust.
This is an exciting opportunity for experienced Mental Health Nurses to join our City LMHT teams. We have vacancies within the City South LMHT.
We will provide the successful candidates the opportunity to develop skills needed to work within a community setting, providing care to an adult population across the Nottingham city area, open to secondary care services, within SMI pathways (mood affective, complex psychosis, community rehabilitation and non psychosis).
You will have the opportunity to access training and develop skills in working within a specific pathway. We offer a very supportive team environment, with access to regular clinical, managerial and peer supervision.
Main duties of the job
The post holder will be expected to hold a caseload and work with patients and their families to support personalised care plans and provide specialist interventions. You will promote the recovery of patients and work with partner agencies and primary care to ensure timely and safe discharge from secondary care.
You will complete both triage assessments and more detailed initial assessments of new referrals. You will also be expected to provide support via the team's duty system, either from telephone support or face-to-face.
This is a leadership role, and you will be expected to develop specialist skills in working with patients in at least one pathway within the team. You will also be expected to support students and more junior staff within the team through clinical supervision and mentorship, and you may be asked to line manage some staff.
We are seeking enthusiastic team players to work within a dynamic multi-disciplinary health team, with excellent communication and interpersonal skills, and the ability and experience to manage complex cases in partnership with other agencies.
Job responsibilities
Day to Day Duties:
* Caseload management for patients living in the community, and acting as care coordinator.
* Triage of referrals, provision of initial assessments and one-to-one interventions with service users.
* To undertake and record comprehensive need/risk assessments, devise and implement treatment plans for service users.
* To demonstrate the application of specialist skills and knowledge in order to maintain professional competence and fitness to practice as a nurse practitioner.
* To establish and maintain robust communication networks with clients, carers, team members, and other health care and social care workers within urgent care and the community.
* To use interpersonal skills in diffusing potentially emotive situations while undertaking assessments and pathway planning within acute settings.
A full UK driving licence and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Person Specification
Qualifications
* Professional Qualification (Mental Health) to degree level or equivalent
* Maintains current registration with the NMC
* Further specialist knowledge/post registration experience in specific service area or other formal training
Knowledge
* Knowledge and understanding of current legislation and understanding of clinical risk assessments
* Knowledge of the nursing 6 C's
* Awareness of national and local issues impacting on nursing care
* Knowledge of Management theories
Skills
* Ability to work as part of a team

Salary: £37,338 to £44,962 a year (pro-rata for part time)
