Registered Nurse, Ward 3, Prince Charles Hospital
We are seeking a dedicated and compassionate nurse to join our 24 bedded care of the elderly/frailty ward. The ideal candidate will provide comprehensive care to older adults experiencing frailty, ensuring they receive personalised, high quality care aimed at improving their quality of life. This role requires strong clinical skills, empathy and a collaborative approach to patient care.
The care of the elderly frailty service is patient centred and meets individual needs to ensure the frail elderly patient is cared for in the right environment at the right time.
The frailty service is a multidisciplinary service for those over 65 years of age who identify with two or more of the five frailty indicators. These include falls, immobility, incontinence, delirium and susceptibility to side effects of medication.
Cwm Taf Morgannwg recognises the NHS Long Term Plan which states that all hospitals with a 24 hour A&E will provide an acute frailty service for at least 70 hours per week. Therefore, the role will develop within the care of the elderly ward and Acute Frailty Unit.
Main duties of the job
You will have the experience of working within a multi-disciplinary setting and have the ability to work within a team of nursing staff. We are looking for registered nurses who are keen to develop skills in multi-agency working, rehabilitation along with complex discharge planning and frailty. We are looking for an individual who has a caring attitude, the ability to be flexible in their approach and capability to adapt. Also important is being able to work as part of a dedicated team, complemented by good communication skills and enthusiasm to learn. We will provide a friendly and welcoming environment and a commitment to continuing professional development within a supportive management structure.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Salary: £30,420 to £37,030 a year per annum pro rata
