Job summary To contribute to the provision of Respiratory Services in Luton to meet the needs of people with respiratory conditions, including COPD, Asthma, ILD and Bronchiectasis who require community services. Able to demonstrate an understanding of the needs of the patient group and will have experience supporting this client group in either a hospital or community setting. You will be able to deliver high quality care and demonstrate clinical decision making, working independently and as part of the wider multidisciplinary team. To be accountable for the management of patients in your care, to ensure a high standard of evidence based clinical care is maintained Ensure all electronic based patient records are kept up to date in line with the Trust's record keeping policy. Work within CCS policy and service specific Standard Operating Procedures. To work in partnership with patients encouraging appropriate use of the resource, including social care agencies and the voluntary sector, raising the profile of respiratory needs across Luton, considering the diversity of population and the promotion of respiratory health. Work with colleagues in the Acute Trust and Primary Care to identify patients who need the support of the multi-disciplinary team to prevent frequent hospital admissions. To work within current clinical guidelines and evidence based practice to develop a gold standard service.
