We are recruiting for an experienced Practice Nurse to join our Nursing Team and co-ordinate the programme of care for our diabetic patients.
Our Practice Nurses are supported by our Lead Nurse and deliver nursing services within the boundaries of their scope of practice to the practice population. The Practice Nurses are responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure. They work collaboratively with our teams to meet the needs of patients and provide excellent and safe patient care. Our nursing team work across 3 of our patient-facing sites within Waterlooville.
We are looking for a motivated team player who is highly organised, enthusiastic and patient-focused with a can-do attitude and friendly personality. Candidates must have evidence of Diabetes training (Diploma, PitStop or other accredited course).
We can consider candidates seeking 30 - 37.5 hours per week between Monday- Friday, with a working pattern to be agreed. Extended hours on a rotational basis will form part of the working pattern (after successful completion of the probationary period).

Job responsibilities
* Assist and perform tasks related to patient care as directed by Lead Nurse and GPs following agreed clinical protocols.
* Deliver evidence-based care and structured education to people with diabetes, their families and other health colleagues in a primary care setting. This includes participating in joint clinics and domiciliary visits for patients with diabetes including those living in Care Homes (who have complex needs) whilst educating primary care staff to enhance their skills to provide an excellent diabetes service.
* Participate in local Diabetes Steering Groups, particularly looking at clinical standards, guidelines, pathways and improving outcome measures.
* Lead the internal diabetes meetings, updating colleagues on new NICE and local guidance and sharing prescribing knowledge.
* Provide holistic assessment of wounds and implement wound care including complex wound management in line with current evidence-based guidelines such as National Wound Care Strategy (Dopplers/Compression).
* Support NAs/HCAs carrying out wound care by maintaining clear care plans.
* Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health, promote patients to live healthily, and encourage principles of self-care.
* Maintain chronic disease registers and assist with QOF searches and reporting.
* Participate in developments within the practice and support QOF requirements.
* Maintain accurate clinical records in conjunction with Practice policies and current legislation.
* Ensure read codes are used effectively.
* Ensure safe storage, rotation and disposal of vaccines and medication. Assist in the completion of weekly stock take and support the cost-effective use of disposable items.
* Support the clinical team with all safeguarding matters, in accordance with local and national policies.
* Provide appropriate self-care advice for patients presenting with minor illness within own level of competency and referring to GP/ANP where appropriate.
* Recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines.
* Perform Cardio-pulmonary Resuscitation and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines.
* Manage emergency drug kit and perform weekly checks as appropriate.
* Undertake the collection of pathological & microbiology specimens including venepuncture, swabs, urine culture in line with local guidelines.
* Travel Health Consultations.
* Womens Health including Cervical Screening.
* Provide contraception advice and sexual health guidance. Undertake annual contraceptive reviews and injections in line with practice policies.
* Offer advice about childhood and adult immunisations and ensure vaccines/injections are administered under patient group directives (PGDs) or individual prescriptions (PSDs).
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required.
* Deliver opportunistic health promotion.
* Chaperone and assist patients where appropriate who are being examined by another clinician.
* Provide information and advice on prescribed or over-the-counter medication, on medication regimens, side effects and interactions as appropriate and within your level of competency.
* Request pathology tests, for example bloods, microbiology, swabs.
* Process pathology results as required and act on them appropriately.
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Actively participate in all aspects of infection prevention and control and champion this within the practice.
* Maintain and clean equipment and clinical rooms and re-stocking/rotate items as required.
* Attend and participate in practice meetings as required.
* Actively take part in mandatory and internal/external training as requested.
Person Specification
Experience
* Experience as a Practice Nurse in Primary Care, following agreed clinical protocols with referral to GPs as appropriate.
* Experience of managing Diabetes as a primary condition.
* Experience of chronic disease management.
* Post graduate experience in one or more areas of chronic disease.
Knowledge & Skills
* Clinical skills and knowledge: Wound care including compression bandaging and ABPI, Urinalysis, ECGs, Venepuncture, Hypertension, Cardiovascular disease primary prevention, Coronary Heart Disease, Diabetes, Routine immunisations/childhood immunisations, Requesting pathology tests, for example urine culture, swabs, Cervical Screening.
* Excellent organisational and time management skills.
* Ability to work as a team member and autonomously.
* Excellent communication skills, able to effectively engage with a diverse range of people facilitating differing communication styles and needs.
* Full UK Driving licence with own transport.
* Sound IT skills with a working knowledge of Windows and Office 365.
* Working knowledge of EMIS.
* Understanding of QOF.
* Clinical skills and knowledge: Able to remove coils, Ring pessaries, LARC fitter, Travel medicine, Spirometry.
Qualifications
* NMC Registered Nurse.
* Level 6 qualification or above in Diabetes Care / Management.
* Post graduate diploma or degree (in one or more areas of chronic disease).
