An opportunity has arisen for a mental health practitioner to join us in the Memory Assessment Service (MAS).
We will provide you with the opportunity to develop transferrable skills - to become specialised in the assessment process by further developing your assessment techniques whilst carrying out in-depth first line assessments; collecting and collating information in an appropriate format to present to your MDT colleagues; sharing and discussing sensitive information as well as participating in the diagnostic process; providing knowledge and support during those early days of diagnosis as well as being involved with initial therapeutic group/individual work. We want to support you in your career and give you opportunities to progress.
The role will require a mix of nursing skills from a range of professional specialist areas - Primary Care, Liaison, Adult Mental Health and Older Person's Mental Health. As the B6, you will be expected to lead an area of the MAS in the absence of the manager. This is a unique opportunity to join our team during these interesting times of change.
Main Duties of the Job
The successful candidate will be a part of the Memory Assessment Service where they will work with those who are experiencing cognitive changes and have concerns regarding Dementia. The successful candidate will undertake responsibility for the Memory Assessment Service in the absence of the manager. They will be required to divide their time equally between a clinic and community care setting due to the large rural geographical area with limited transport links and problematic mobile phone reception. It is necessary to work autonomously and conduct patient home visits. The successful candidate is expected to participate as a full member of the multi-disciplinary team, working with people who present with possible organic mental health problems. Taking responsibility for a caseload of patients and initial comprehensive assessments, delivering diagnosis and formulating treatment plans and initial reviews.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
