Job overview
An exciting opportunity has arisen to join the Home First Therapy team in Buckinghamshire. The team is still in its infancy of becoming a substantive service and this provides team members with the opportunity to influence and contribute to growing and shaping the service. We are looking for motivated and skilled individuals to join the team, which supports patients that have recently been discharged from hospital, to regain their independence and remain in their own homes.
The ideal candidate will be able to take a holistic and person-centred approach to supporting patients to recover from their recent hospital admission, and to achieve their short-term goals, maximising independence. The role will include visiting patients in their home, providing therapeutic treatment of patients, under the guidance and supervision of qualified therapists within the team.
Main duties of the job
* To be an integral member of the Home First Therapy team, providing prompt assessment and delivery of therapeutic treatments to patients in liaison with senior Therapy staff.
* To take responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients' own home.
* To act as a technical resource to the Home First Service, in relation to the provision of mobility equipment.
* To document, evaluate and liaise with qualified Physiotherapists and Occupational Therapists following interventions.
* To be responsible for the completion and potential delegation of non-clinical tasks within Home First therapy team.
* To work as part of the weekend Home First Service.
Person specification
Education, Qualifications & Training
* NVQ level II or III in Health Care
* Commit to undertaking Foundation Degree and any associated training once available
* Manual Handling, or First Aid qualification
Experience
* Significant experience working within a patient-facing environment with a reablement/rehabilitation focus.
* Previous experience working with patients with physical, cognitive or behavioural impairment
Skills, Abilities & Knowledge
* Clear, concise verbal and written communication to ensure liaison with multi-disciplinary team, patients and carers to ensure records and reports are accurate, legal and in legible manner (in English).
* Collaborate with multi-professional service users, internal and external at all levels.
* Able to motivate self and be able to work with all levels of staff.
* Ability to work well in a team and independently.
* Able to accept and undertake responsibility for designated tasks.
* Demonstrate good organisational skills.
* Demonstrate insight into the importance of maintaining a safe working environment.
* Ability to learn/ability to self-initiate learning (evidence within the last 2 years).
* Able to know own limitation and seek advice.
* Able to complete tasks set, follow instructions.
* Physical ability and endurance work in the physical environment of the clinical area required by this post.
* Able to work under pressure and to tight deadlines.
Special Circumstances
* Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.
* Ability to work at weekends/Bank Holidays.
