Job Summary:
The Providence Center Manager Integrated Community Treatment Team (ICTT) Prairie Avenue will supervise the daily operations of the ICT Team, serving the chronically homeless and co-occurring population which requires high-level services, as well as perform direct service duties when applicable. Develop and create a continuum of care for consumers, divert consumers from the hospital when clinically appropriate, assist in timely step down services to those hospitalized, and focus on resources and access to available housing and shelter.
The Manager ICT Prairie Ave Team directly supervises all employees on a team of up to 13 staff, in conjunction with the Director. Responsibilities include planning, assigning, and directing work; addressing complaints and resolving problems.
Duties and Responsibilities:
1. Supervise the daily operations of the team.
2. Ensure the team adheres to the evidence-based ICT Team model of care.
3. Coordinate consumer care; oversee consumer admissions to and discharge from the hospital and the admissions of consumers to the team.
4. Conduct regular supervision meetings with staff and maintain records of the meetings per BHDDH requirements.
5. Provide leadership to staff regarding training, risk management, day-to-day matters, etc. Facilitate an environment that supports staff participation and encourages self-direction and team building.
6. Facilitate treatment planning meetings for the development, review, and implementation of person-centered, recovery-oriented treatment plans, in a timely manner.
7. Ensure that confidentiality and the highest standards of ethics are maintained by staff when treating consumers, in accordance with Center policy and legal requirements.
8. Ensure the staff's quality and timely completion of Health Information Services (HIS) and other required documentation.
9. Perform administrative functions in a timely manner and in accordance with Center policies; interviewing prospective employees, disciplining staff, completing performance evaluations, oversight of petty cash, etc.
10. Develop staff schedules to assure on-call and adequate coverage to meet consumer needs.
11. Maintain billable hour targets and caseload statistics. Ensure staff compliance with standard.
12. Provide orientation and training to new staff.
13. Assist with ongoing training and staff development.
14. Conduct record audits of staff records in accordance with Center requirements. Establish plans of improvement as needed.
15. Ensure that staff participating on committees regularly attend meetings and report back to the department on committee activities.
16. Serve as a member of the ICT Team Services Management Team to develop programs, in-service trainings, solve problems, and generate ideas and to implement Center policies.
17. Carry a small caseload when needed and provide direct service.
18. Provide support counseling, problem-solving, contracts, and limit setting.
19. Establish attainable goals with the consumer.
20. Encourage consumers to attain the highest possible levels of independence.
21. Accept the values and lifestyles of consumers.
22. Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, including working with substance abusers, as it relates to case management.
23. Complete assessments and provide appropriate crisis interventions and risk management as needed.
24. Teach consumers adult daily living skills.
25. Initiate treatment and advocate for consumers within the network of care.
26. Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
27. Serve as liaison with Providence Center therapists, programs, and community agencies that are also providing services to the consumer.
28. Complete record keeping in accordance with Health Information Services and other Center requirements.
29. Transport consumers in own vehicle or Center van to planned activities and appointments.
30. Maintain cooperative relationships with other employees, consumers, and the public.
31. Attend trainings and conferences.
32. Perform duties in accordance with departmental goals, objectives, and standards.
33. Attend meetings and report committee activities to staff.
34. Other duties as assigned.
Requirements:
1. Master's degree in a clinical area required.
2. Eligibility for QMHP required.
3. Minimum 3+ years’ experience working with psychiatrically disabled consumers in a community support setting, preferably in a Supervisory role, and knowledge of the special needs of the deinstitutionalized consumer, knowledge of BHDDH rules and regulations, and community resources.
4. Demonstrated success in program development relating to Integrated Community Treatment Teams.
5. Valid driver's license and insured automobile required.
