Job summary An exciting opportunity has arisen to support stroke survivors through the stroke care pathway in the London Borough of Harrow, which is home to one of the busiest Hyper-acute stroke services in the UK. The post holder would be working closely with stakeholders across the acute trust, ICB, social services and the independent sector to ensure patients receive seamless care from discharge from the acute or rehabilitation phase. The post holder will be supported by the Stroke Unit Clinical Manager based at Northwick Park Hospital, and the Team Manager in Harrow Adult Social Services. The post holder will be leading on the collection of data of 6 week and 6 month assessments of stroke survivors and the inputting into the Sentinel Stroke National Audit Programme (SSNAP). The post holder should have advanced knowledge and experience of working with stroke survivors and of the stroke care pathway. They should also have advanced communication skills and experience of communicating with patients with communication difficulties. This role will enable the post holder to develop their skills in leadership and service development. The role involves hybrid working, between home, Northwick Park Hospital and Harrow Civic Centre. You will be joining a vibrant and diverse team that has a high level of experience in managing specialist stroke services. Main duties of the job To identify and co-ordinate the Stroke Care Pathway for all adult stroke patients in the London Borough of Harrow, working across the Acute Trust, CCG, Social Services and the independent sector. To ensure that patients receive seamless care from discharge from the acute / rehabilitation phase and their return to the community. To liaise effectively with key front line practitioners and across the Acute Trust, Rehabilitation and Intermediate Care Teams, District Nursing, GPs, Nurse Consultant for Older people, Social Services and the independent sector. To ensure patients receive appropriate review after discharge from an inpatient setting. Establish a regular stroke club/stroke aftercare group in a community setting for stroke, survivors, family members and/or carers to attend. This will focus on models of best practice. To ensure stroke patients are signposted to appropriate support groups and voluntary organisations. To ensure secondary prevention and support for stroke patients in the community following their initial episode of care. To keep abreast of evidence-based practice and most recent research publications in order to ensure the highest clinical standards and up to date practice. To provide effective clinical leadership to support and inspire colleagues to improve standards of care and promote continuous improvement in the management of stroke and complex neuro conditions.
