Role Summary
The hospital services team in collaboration with Devon and Cornwall carer services, work with the hospital staff to identify carers to offer support and information to help make their caring role more sustainable for the future. We work to help avoid admissions and re-admissions and to support hospital staff to be carer aware.
About the role
We are seeking a committed, organised, and self-motivated person to provide support to carers in hospital setting.
This will be achieved by:
Provision of support to unpaid carers via information, advice and signposting, including administration of small grants to aid discharge and prevent re-admissions.
Raising awareness of unpaid carers with health professionals across multiple disciplinary teams.
Partake in ward rounds at Derriford hospital to identify and signpost carers to their local carers support network.
The postholder will work closely with Improving Lives Plymouth colleagues and various health and social care professionals, including hospital staff, adult social care and Plymouth City council to ensure carers receive timely and appropriate support.
This exciting opportunity requires a combination of excellent person-centered support to those with caring responsibilities, relationship development, communication and organisational skills and the ability to work with a range of organisations across Plymouth.
Responsibilities
You will be required to meet contractual and grant requirements by delivering information, advice, and support services in a one to one or group setting.
You will work closely with volunteers to enhance service delivery.
You will engage with service users on a formal and informal level to ensure they are involved in the coproduction of service delivery.
You will be required to work closely with your line manager to contribute towards developing plans, monitoring, evaluating, and producing regular reports.
You will be accountable for the use of charitable funding.
You will be an active member of the Improving Lives Plymouth team working closely with peers to develop cross project delivery to provide a wraparound service to our service users reducing the need for them share their story when moving between different teams within the organisation.
You will be required to assist with the implementation of identified accreditation standards.
You will be required to undertake any additional training relevant to the role to contribute towards your continued professional development.
