The role will provide business and administrative support to the Transfer of Care Hub (TocH) and their team members. Main duties of the job We are looking for an enthusiastic and motivated individual who will support the Transfer of Care Hub lead on the daily escalation calls, produce reports/data as required and maintain and coordinate the dashboard for the discharge hub, collating updates, actions and comments from the multidisciplinary team (MDT).
