An exciting opportunity has arisen to join our Primary Care Network social prescribing team. CHOC primary care network is looking to recruit a forward thinking and enthusiastic social prescribing link worker to join our established social prescribing team. Our PCN area covers practices across Congleton and Holmes Chapel (CHOC) Primary Care Network. We are seeking to appoint a flexible minded social prescriber to take a front line role working with the practices. This role will help empower patients to take control of their health and wellbeing, allowing them to create time to focus on what matters to them most. Social prescribing empowers people to take control of their health and wellbeing through referral to link workers who give time, focus on what matters to me and take a holistic approach to an individuals health and wellbeing, connecting people to diverse community groups and statutory services for practical and emotional support. This role will help a range of patients, some with long term conditions manage their health differently and more effectively or help people who are lonely or isolated, or who have complex social needs which affect their wellbeing, including their mental health. You should be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of working positively with people facing complex social and emotional challenges. The successful candidate should expect to work across all the practices delivering care to patients from a wide range of backgrounds.
