Service Provision Lead on the improvement of medicines management and optimisation in all clinical areas within the trust. Identify and implement methods of improving medicines use to reduce in-patient length of stay, improve discharge, and make the best use of hospital resources. Work closely with other healthcare professionals across the Trust to develop policies and procedures to optimise the use of medicines. Collaborate with pharmacy, medical, and nursing staff to reduce medication-related adverse events and formulate action plans with healthcare groups to reduce errors and risks related to the use of medicines. Support wards in ensuring the safe and secure storage of medicines and maintain monitoring of temperatures in areas where medicines are stored. Assist in improving safe discharge processes and ensure the best medicines-related learning experiences for non-medical staff. Support ward pharmacists in developing and reviewing clinical guidelines, policies, and SOPs. Work with the pharmacy ePMA team as part of the Trust EPR roadmap, leading audits where required. Risk assess all wards/departments to ensure safe preparation of medication. Engage in efforts to improve patient knowledge about their medication. Work with pharmacy, medical, and nursing staff to reduce drug wastage. Function as an Independent Non-Medical Prescriber within a designated clinical area, managing patients' clinical assessments and prescribing as needed. Education, Training & Audit Ensure an effective learning environment for medicine management in all wards/departments. Develop and provide training on safer discharge processes, FP10 prescriptions, and TTA pre-packs, while maintaining training records. Manage systematic, multidisciplinary evaluations of medication incidents, ensuring risks are assessed and graded according to Trust policy. Collaborate with clinical directors, managers, and matrons to enhance reporting and learning from medication safety incidents. Identify and deliver training for healthcare professionals to support the risk management agenda in medicines management and safety. Participate in Pharmacy Clinical Governance Meetings, integrating agreed plans into the overall Quality Management System within Pharmacy. Ensure pharmacy service developments at ward and departmental levels are assessed for potential risk management issues using tools like FMEA. Support the development and review of Patient Group Directions (PGDs) and maintain a timely audit and review database. Organise and deliver training programs on MSC-led topics such as opioid stewardship and discharge planning. Assist ward managers in maintaining risk assessments for safe medication practice.
