An exciting opportunity to has become available in the Healthy Knowsley Service, within our Children Families Team. This is an opportunity for someone who is passionate about reducing health inequalities and increasing health aspirations.
This will be an opportunity to provide in person interventions to families at risk of unhealthy weight. The postholder will engage with school communities to identify and enrol eligible families.
The postholder will maintain a caseload of children and families, under the direct/indirect supervision of a Senior Health Improvement Practitioner. They will deliver interventions that increase knowledge, skills and confidence to adopt health promoting behaviours. Healthy lifestyle messages will be delivered through activity based learning with an emphasis on being active, learning and having fun as a family.
The post holder will have a good knowledge of factors that impact overweight and obesity in children. They will have experience of facilitating engagement in health related activities for families. They will be skilled at adapting physical activity based sessions to ensure engagement.
The post holder will work as part of the Healthy Knowsley Service to support families and children to make sustainable changes to reduce health inequalities across the Borough. This will include the delivery of high quality interventions that: provide a positive experience of physical activity; increase activity levels in the whole family; increase knowledge of healthy eating messages; improve cooking skills and support sustainable changes in health related behaviours.
Interventions will be delivered in school and community environments and virtually.
The post holder will maintain accurate, clear, concise and contemporaneous records as per trust policy and communicate information to members of the Team as necessary.
The post holder will provide concise handover and information updates to other members of the Team as required.

MAIN RESPONSIBILITIES
1. Meet with families to assess lifestyle risk factors and support them to develop relevant
lifestyle improvement initiatives.
2. Provide support, motivation and development of appropriate action plan for each
family.
3. Carry out review meetings to assess progress and provide long term motivation and
guidance.
4. Deliver healthy lifestyle programmes to school communities.
5. Deliver multi component interventions to families incorporating goal setting, physical
activity, healthy eating and cooking skills.
6. Monitor attendance and achievements of clients on courses or activities recording and
documenting as appropriate
7. Organise and facilitate groups and/or individuals to develop self-care skills.
8. Whilst working with families and schools, identify barriers and possible solutions to
making sustainable lifestyle changes, reporting these back to the Team Manager.
9. Support the development of new programmes within the Healthy Knowsley Service.
10. Support the development of new lifestyle initiatives which will offer peer support to
clients trying to maintain lifestyle changes.
11. To assist the Healthy Knowsley Service to continually review the development and
improvement of the range of services provided.
12. To contribute to evaluation/research initiatives designed to generate data regarding the
effectiveness of the service.
13. Participate in community events and meetings as required.
14. Support promotional activities and events for the service.
15. Participate in mentorship and clinical supervision within the Healthy Knowsley Service
16. Provide day to day supervision of Apprentice.
17. Be responsible for co-ordination of a named designated project.
18. Produce progress or activity reports as required in relation to service.
19. To support Healthy Knowsley service and its interests, and other public health teams as
required at local and regional events.
20. Alert line manager to any issues in work regarding specific areas of concern which may require specialist support including concerns about individuals or work in the community.
21. Liaising with other team members in the production of reports, newsletters, and contact with the media.
22. Undertake any other duties that may be required and which are applicable to the grade of the post.
23. To plan and organise time effectively to meet the needs of the individuals and the service.
24. To understand and apply Health & Safety requirements in line with Trust Policy & Procedures.
LEARNING AND DEVELOPMENT
1. To complete induction training followed by a vocational training
Programme.
2. To complete Care Certificate
3. To participate in personal appraisal and learning activities.
CLINICAL GOVERNANCE/RESEARCH/AUDIT
1. To participate in research and audit relating to the use and effectiveness of the service.
2. Ensure all information is collected in accordance with Data Protection Guidance.
COMMUNICATION
1. To help to build up strong links between the service and all organisations and other
interested statutory sector partners.
2. To communicate with other colleagues as part of the service and with other health and
social care professionals.
3. Required to operate as part of a team, sharing information and knowledge and learning
4. To be responsible in developing and contributing to team support networks and join in
team learning activities
5. Communicate information to individuals about health and well-being and the relationships between behaviours and health
6. Facilitate and support individuals to develop their knowledge and skills about health and well-being
7. Build and maintain knowledge of key contacts within the community
8. Promote the equality and value the diversity issues and link to the relevant partners
