We are also looking for a Support Worker role (Support Worker - £24,000) These roles will be covering North and Central Nottingham, Mansfield and Ashfield and North Newark, Sherwood and Bassetlaw. Moving Forward is a floating support service for people with mental health problems covering Mansfield, Ashfield Newark & Sherwood and Bassetlaw districts, working in partnership with local Community Mental Health Teams. We are looking for an enthusiastic, compassionate Support Planner who would thrive in a community-based service where no two days are ever the same. The Moving Forward service has a long and successful record of supporting people to achieve and make key, positive changes to their own lives. If you join our dynamic and motivated team, you will carry your own caseload and support your colleagues to excel at all times. Ideally you should have a working knowledge of the support needs of people with mental health problems living independently in their own home but being able to demonstrate transferrable skills and a commitment to continuous learning would be acceptable. Extensive training and induction will be provided to the right candidate. In this role you will plan and provide specifically tailored support to help people to achieve their individual goals and assist them to review and learn from their progress and achievements. You would be expected to keep accurate and relevant support notes, demonstrate how to resolve problems, advocate and communicate at all levels with various community agencies.
