An opportunity has arisen for a highly motivated and enthusiastic Band 6 nurse to join the Pancreas Transplant Nurse Specialist Team at the Oxford Transplant Centre. You will be based in Oxford in an outpatient setting, and the role involves covering peripheral outreach clinics, so a full current driving license is essential. You will be working as part of a MDT and, after appropriate training, as an autonomous practitioner.

The post holder could have a renal nursing background or experience in caring for people with long-term health needs.
Within three months of starting this post, and following a supportive period of training, you will join the recipient coordinator on-call rota, providing a 24hr service, including weekends and bank holidays, coordinating all transplant offers for Oxford recipients.
Main Duties of the Job
* Accept patient referrals to agreed criteria and be identified as the key worker for an agreed patient/client caseload.
* Provide advice, education, and support to patients, their carers, and other healthcare professionals.
* Practice clinically in agreed care settings according to approved protocols or guidance.
* Contribute to the smooth running of the designated service.
* Liaise with other hospital and community teams along an agreed pathway to support the delivery of patient and family-centered seamless care.
* Contribute to the ongoing development of the designated service.

Job Summary: The Pancreas Transplant Nurse Specialist role is to assist in the coordination and management of patients within the Oxford Transplant Centre that are undergoing assessment and work-up for a Pancreas transplant. The role involves conducting patient consultations addressing clinical, psychological, and social needs under the supervision of the Lead Pancreas Transplant Nurse with clinician support. This may include clinics during the week and occasionally at weekends in outreach and referring centres as well as within OUH. The post holder also works within the wider Transplant Specialist Nurse team providing on-call transplant recipient coordination 8am-8pm and at weekends (occasionally 24 hours).
