We are seeking a compassionate and dedicated Social Worker to join the Adult Community Team. As a vital member of the team, you will be playing a key role in providing essential support and services to adults in the community, empowering them to lead fulfilling lives and promoting their overall well-being.
Responsibilities include, but are not limited to:
1. Conducting comprehensive assessments of individuals' social care needs, considering physical, emotional, and environmental factors.
2. Developing and implementing tailored care plans in collaboration with service users, promoting independence and resilience.
3. Providing crisis intervention and support during challenging situations, working closely with other professionals and agencies.
4. Working collaboratively with healthcare professionals, local authorities, and community organisations to provide holistic care.
5. Attending multi-disciplinary meetings, contributing valuable insights and expertise to enhance the overall care and support network.
6. Fostering positive relationships with external agencies to strengthen the support available to service users.
7. Maintaining accurate and up-to-date records of assessments, interventions, and service user progress.
8. Preparing reports for case conferences, reviews, and legal proceedings as required, ensuring compliance with data protection and confidentiality policies.
Requirements:
1. Qualified Social Worker with a recognised Social Work degree.
2. Previous experience in adult social care and community-based settings.
3. Strong knowledge of relevant legislation, policies, and procedures.
4. Excellent communication and interpersonal skills.
5. Ability to work independently and as part of a multidisciplinary team.
6. Commitment to maintaining professional boundaries and promoting ethical practice.
