Job summary
An exciting opportunity has arisen to join our Cardiology Outreach / Liaison ANP team at the OUH, to join a service provided by our team of highly valued Cardiology Liaison Advanced Nurse Practitioners. The successful candidate will be required to work across our two acute hospital sites, the John Radcliffe Hospital in Oxford, and the Horton General Hospital in Banbury to provide Cardiology outreach cover 7 days a week as part of a team.
We are looking for an expert nurse with a strong interest in Cardiology who is a team player, and who has the ability to lead and develop practice in order to provide a high quality service throughout the patient pathway for patients and their families, working with clinical colleagues to provide information, advice and support to patients with cardiac conditions.
Main duties of the job
The Post holder will work as part of the Cardiology team to provide advanced clinical assessment, support and onward referral for patients attending the Emergency Department, Emergency Assessment Unit, and any department or ward in the OUH sites. Undertake to assess patients presenting in the Rapid Access Chest Pain clinic.
The aim of the post holder is to expedite assessment, including appropriate tests and investigation based on clinical need and to provide quality of care and support for patients and their families.
The Post holder will liaise across organisational boundaries with the health care teams in primary care, secondary care and tertiary care to establish systems of care that support the referral and management of patients that have or require urgent admission with a range of Cardiology conditions.

Job responsibilities
The Post holder will work as part of the Cardiology team to provide advanced clinical assessment, support and onward referral for patients attending the Emergency Department, Emergency Assessment Unit, and any department or ward in the OUH sites. Undertake to assess patients presenting in the Rapid Access Chest Pain or Outpatient cardiology clinics within the Oxford University Hospitals NHS Trust with cardiology conditions.
The Post holder will provide and facilitate the delivery of multidisciplinary care for cardiac patients at the OUH. The post holder will contribute to the development of cardiac services at a local and national level, providing nursing expertise for patients, families and carers.
