Day Support Worker
Location: Heathy House, Halifax, West Yorkshire
Salary: Currently £23,712 increasing to £24,897.60 per annum - From April 2025
Hourly Rate: £12 - £12.60 per hour
Hours: Full-time, 38.5 hours per week
Shift Pattern: 8am to 8pm on a two-week rolling rota
1. Week 1: Monday, Tuesday, Saturday, Sunday
2. Week 2: Wednesday, Thursday, Friday
Active Pathways Ltd is currently recruiting for compassionate Day Support Workers to join the dedicated team at Heathy House in Halifax, West Yorkshire. This is an exceptional opportunity to work as an integral part of the team, ensuring the individuals we support can live as independently as possible.
As a Support Worker, you will:
* Support individuals on their journey to recovery through person-centred care in a rehabilitation setting.
* Help residents develop independent living skills and achieve personal goals.
* Work within a multidisciplinary team to build positive relationships with residents.
* Provide emotional and practical support to residents.
* Promote engagement in meaningful activities and encourage social inclusion.
* Assist with daily living tasks and support residents in accessing education, employment, and the community.
* Promote independence and self-confidence, helping residents regain control over their lives.

EXPERIENCE:
* Experience within a Mental Health or Rehabilitation setting
* Can build rapport with ease with not only the residents but the wider team
* Understanding the importance of offering person-centered quality care
MAIN RESPONSIBILITIES:
* Working seamlessly within the multidisciplinary team
* Supporting residents with activities
* Continually promoting independence
