Responsibilities
Assessing the needs of adults within the community and developing personalized care plans.
Monitoring the wellbeing of clients and providing ongoing support to meet their needs.
Collaborating with other healthcare professionals and community organizations to coordinate care.
Conducting regular reviews of care plans and adjusting as needed based on client feedback and changing circumstances.
Providing emotional support and practical assistance to clients and their families.
Maintaining accurate records of client interactions and progress in line with compliance requirements.
Staying informed about local resources and services to assist clients effectively.
Requirements
Relevant qualifications in health or social care, such as a Level 2 diploma in health and social care.
Experience working with adults in a community care setting or similar environment.
Strong communication and interpersonal skills to build rapport with clients and their families.
Empathy and compassion for individuals facing various challenges and vulnerabilities.
Ability to work both independently and as part of a team in a fast-paced environment.
Knowledge of safeguarding policies and procedures related to adult care.
Excellent organizational skills to manage multiple cases and priorities effectively.
Hours Per Week: 37.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £15.70
Location: Lincolnshire, East Midlands
